Housing & Care for Households with Mental Health Challenges and Disabilities
The Continuum of Care Housing Program (CoC) is a 34-unit permanent supportive housing program for homeless families with mental illness and/or disabilities. The program helps participants obtain/remain in permanent housing, increase their skills and/or income, and achieve greater self-determination.
Operating under contract with the Behavioral Health Administration (BHA) through the Prince George’s County Health Department’s Behavioral Health Division, this project-based rental assistance program is HUD-funded.
Eligibility
Individual and family applicants are referred or self-referred and are processed through the Continuum of Care coordinated assessment and entry system. To be admitted to the program, all applicants must:
- Be a U.S. citizen or a legal resident of the U.S. and at least 18 years of age
- Be homeless for one year within a four year period
- Receive less than the median income for the area
- Have a completed service plan identifying appropriate services, supports and activities
- Meet legal criteria for the program. Applicants charged or convicted with misdeameanor or nonviolent felonies are eligible
- Meet the HUD definition of homelessness (please visit HUD.gov for more information)
- Be in need of mental health and supportive services and participate in services
Additionally, participants must have a serious and persistent mental illness, as identified by the following diagnostic categories:
- Schizophrenia
- Major Affective Disorders
- Delusional Disorder
- Psychotic Disorder, NOS
- Bipolar Disorders
- Schizotypal / Borderline Personality Disorder
Services Provided
Comprehensive supportive services are provided by our Community Psychiatric Rehabilitation (PRP) and Targeted Case Management (TCM) programs, and include:
- Permanent supportive housing
- Rent & utilities assistance
- Housing counseling
- Landlord mediation
- Eviction prevention
- Case management / psychiatric rehabilitation
