Community Psychiatric Rehabilitation Program

Located in Montgomery County, PG County, and Baltimore City, MD the program provides services to adults (18 years old and older) with severe and persistent mental illnesses to facilitate the development of their independent living and social skills.

The consumers served in the Community PRP include those in homeless shelters, transitional housing programs, and permanent housing programs. The program is funded through Maryland Medicaid fee for service.

Assigned counselor visits the consumer 5-6 times per month (based on his/her assessed needs) at the residence.

Services available through our program include:

  • Entitlement application, renewal, appointment, appeal
  • ADL skills training (hygiene, grooming, exercise, laundry, money management, representative payee service if necessary, meal preparation, menu planning, maintenance of living environment)
  • Medication compliance, training, obtaining medications
  • Substance Abuse education, referral
  • Socialization training
  • Community awareness, integration
  • Individual advocacy assistance
  • Transportation training
